Proof of Identity

Proof of Identity

To help us identify the person applying for an Australian passport, it is required to provide additional proof of identity by completing the Proof of Identity Declaration (section 12-Guranter's section) of the PC 8 form. The person completing the declaration should be:

- 18 years of age or over

- have known the applicant for the past twelve months (where the application is for a child under the age of 12 months, there is still the requirement to have someone providing the proof of identity; the person should either be a doctor or nurse who can certify that they have known the child since birth)

- not be related to the applicant by birth or marriage

- an Australian citizen who holds a passport of at least two years validity, or a national of another country who is currently employed in one of the selected professional or occupational groups listed below:

  1. Accountants (Registered)
  2. Bank Managers
  3. Barristers, Solicitors and Patent Attorneys
  4. Chartered Professional Engineers
  5. Members currently serving in the regular Defence Force with at least five years continuous service
  6. Dentists
  7. Registered Medical Practitioners
  8. Members of the Parliament
  9. Judges
  10. Public Servants with at least five years continuous service
  11. Teachers - teaching full time for more than five years at tertiary institutions
  12. Registered Veterinary Surgeons
  13. Registered Nurses (SRN)

The Guarantor should complete the declaration on section 12 page 9 of 11 of the application form (PC8), endorse the back of both photographs by writing, "This is a true photograph of (applicant's name in full)” and signing.
