*It is a requirement of the Passports Act 2005 that the written consent to the issue of an Australian passport to an unmarried person under 18 years of age must be provided by each person who has caring responsibility for the applicant (i.e. any person who has parental responsibility including rights of custody, guardianship or access). Consent may not be conditional and is required for the full validity of the passport.
